Transition to Work (TTW)

Assesment Information

How can the young person get an assessment?

Speak to the young person's teacher or careers advisor.  Staff from Essential Personnel can also give you information to assist you in the process.


What is the assessment all about?

The assessment includes a number of standardised screening tools that focus on daily living functioning, behaviour and domestic functioning.  The information collected is forwarded to the Centre for Health Service Department at the University of Wollongong for analysis. Identity of individuals is not disclosed to the University.

Based on the assessment results, ADHC determines the young person’s eligibility for Transition to Work or Community Participation.


When does the assesment take place?

Assessment normally takes place around July and people are notified of their outcome in September.

When do you find out if you have been successful?

In September.


If you are successful, then what?  Then it’s time to choose a service provider.

Each year the Essential Personnel provides a Taste of Transition opportunity.  This allows potential eligible participants to sample what the program offers. The Taste of Transition will include sampling participation in the Becoming a Worker, site visits, guest presentations by a variety of speakers and a tour of the site.

Taste of Transition is generally offered in May, June, July or September.
